The following E-log aims at discussing our patient de-identified health data shared after taking the guardian's signed consent.  Here we discuss our individual patient problems through series of inputs from available global online community of experts with an aim to solve the patients clinical problem with current best evidence based input. This E-log also reflects my patient's centered online learning portfolio. I have been given this case to solve in an attempt to understand the topic of "Patient Clinical Data Analysis" to develop my competency in reading and comprehending class data including history, clinical findings, investigations and come up with a diagnosis and providing treatment best to our skills and wisdom. A 55 year old female, resident of Miryalaguda housewife by occupation came to hospital on 15/9/22 with  CHIEF COMPLAINTS        Itchy lesions all over of the body since 15 years . She complains of fever with pain with appearance of blisters . HISTORY OF PRESENT ILLNESS       The patient was apparently assymptomatic 15 years ago. Then she complains of itching all over her body associated with pain and fever and appearance of blisters. PAST HISTORY       No history of DM       No history of HTN        No history of CAD , Asthma , epilepsy, TB FAMILY HISTORY          A history of DM ,HTN         No History of CAD , TB , Asthma  PERSONAL HISTORY        Married        Farmer by occupation        Mixed diet         Normal appetite        Normal bowel movement        Normal micturition        Non alcoholic        Non smoker        No known allergies    GENERAL EXAMINATION         Conscious and coherent        Moderately built        Moderately nourished         No pallor        No icterus    

Hi, I am Tejaswi Uppalapati, 3rd semester medical student. This is our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. This elog also reflects my patient centered online learning portfolio.   A 21 yr old female housewife by occupation came with  Chief complaints : 1. Fever since 4 days 2. Vomiting 2- 3 episodes/day since 3 days 3. Body pains since 4 days HOPI : Patient was apparently asymptomatic 4 days back then she developed fever, more in the evening associated with chills relieved with medication. Then patient started having vomiting since 3 days 2-3 episodes per day with food as contents, non-projectile, non-bilious and not stained with blood. Patient had body pains since 3 days which was more in lower back, hands and legs which was not relieved with medication. No h/o headache No h/o diarrhea, vomiting. Past History : Not a k/c/o DM,htn,tb,asthma,epilepsy  On presentation: Pt conscious,coherent,cooperative,well oriented

CHIEF COMPLAINT 82 year old male patient , resident of muthkur came to opd with a chief complaint of fever since 5 days and involuntary movements in upper and lower limbs. HISTORY OF PRESENT ILLNESS   Patient was apparently asymptomatic 2 days back. In the afternoon he developed a high grade fever followed by involuntary movements in all the limbs. Tongue biting and uprolling of eye were also there. Was taken to hospital and medications were given. But later that night he developed those symptoms again. He had tongue biting, uprolling of eyes, involuntary micturition and Bowel movements with postictal confusion for around 5 minutes There is no history of similar complaints in the past Initially after regaining consciousness he couldn'
